The 245D program is Minnesota's licensing framework for home and community-based services (HCBS) that support people with disabilities. At Life Fountain, we provide these services with person-centered care, respect, and dignity, in Spanish and English throughout the Twin Cities metro area.
What services does 245D cover?
245D services fall into two categories based on the level of support a person needs:
- Basic supports: help with daily living, home and community skills, companionship, and respite services.
- Intensive supports: more specialized services for people with greater support needs.
Our services under 245D
- Homemaker — support with household tasks
- ICLS / IHS — support for independent community living
- Overnight supervision
- In-home respite to give family caregivers a break
Who is it for?
For children and adults with physical or developmental disabilities or chronic conditions who are enrolled in a Minnesota waiver. We coordinate directly with your case manager to make everything easier.
