Do you know someone who needs support to live safely and independently at home? Send us a referral and our bilingual team takes care of the rest. We work with case managers, social workers, hospitals, clinics, and families throughout the Twin Cities metro area.

- Older adults who need help with daily activities
- People with physical or developmental disabilities
- Children and adults with chronic conditions
- People who qualify for CFSS (formerly PCA), 245D or FMS
- Families who prefer care in Spanish and English
How the process works
- Submit the referral. Fill out the form below or call us at (651) 403-6034. We only need basic contact information.
- We reach out. Our team contacts the person or family within 24 to 48 hours to explain services and answer questions.
- Assessment and plan. We coordinate the necessary assessment and verify eligibility (Medical Assistance or other programs).
- Care begins. We assign a caregiver who fits the person's needs, language, and schedule.
What do you need to refer someone?
No complicated paperwork is needed. A name, phone number, and a brief description of the need is enough to get started. We guide you through the rest of the process.
