CFSS (Community First Services and Supports) is the Minnesota program that replaced PCA (Personal Care Assistance). It gives you more control over who cares for you and how that care is managed, so you can live safely and independently in your own home. At Life Fountain, we serve you in Spanish and English throughout the Twin Cities metro area.

What does CFSS include?

  • Help with daily activities: bathing, dressing, eating, and getting around
  • Support with care-related household tasks
  • Companionship and supervision according to each person's plan
  • The option to hire family members as caregivers (including spouses or parents of minors)
  • Personal Emergency Response System (PERS)

Who qualifies for CFSS?

To access CFSS, you need Medical Assistance (MA), the Alternative Care (AC) program, or, in certain cases, MinnesotaCare. An assessment is required to determine your needs. Our team helps you understand and coordinate that entire process, step by step.

Two service models — you choose

  • Agency model: Life Fountain handles hiring and supervising the caregiver for you.
  • Budget model: you receive a budget and act as the employer, with direct control over who cares for you.
