At Life Fountain, we provide these services with person-centered care, respect, and dignity, in Spanish and English throughout the Twin Cities metro area.<\/p>\n<h2 class=\"wp-block-heading\">What services does 245D cover?<\/h2>\n<p class=\"wp-block-paragraph\">245D services fall into two categories based on the level of support a person needs:<\/p>\n<ul class=\"wp-block-list\"><li><strong>Basic supports:<\/strong> help with daily living, home and community skills, companionship, and respite services.<\/li><li><strong>Intensive supports:<\/strong> more specialized services for people with greater support needs.<\/li><\/ul>\n<h2 class=\"wp-block-heading\">Our services under 245D<\/h2>\n<ul class=\"wp-block-list\"><li>Homemaker \u2014 support with household tasks<\/li><li>ICLS \/ IHS \u2014 support for independent community living<\/li><li>Overnight supervision<\/li><li>In-home respite to give family caregivers a break<\/li><\/ul>\n<h2 class=\"wp-block-heading\">Who is it for?<\/h2>\n<p class=\"wp-block-paragraph\">For children and adults with physical or developmental disabilities or chronic conditions who are enrolled in a Minnesota waiver.
