Send us a referral and our bilingual team takes care of the rest. We work with case managers, social workers, hospitals, clinics, and families throughout the Twin Cities metro area.<\/p>\n<h2 class=\"wp-block-heading\">Who can we help?<\/h2>\n<ul class=\"wp-block-list\"><li>Older adults who need help with daily activities<\/li><li>People with physical or developmental disabilities<\/li><li>Children and adults with chronic conditions<\/li><li>People who qualify for <a href=\"https:\/\/lifohhc.com\/en\/cfss-cuidado-personal-minnesota\/\">CFSS (formerly PCA)<\/a>, <a href=\"https:\/\/lifohhc.com\/en\/servicios-245d-minnesota\/\">245D<\/a> or <a href=\"https:\/\/lifohhc.com\/en\/servicios-fms\/\">FMS<\/a><\/li><li>Families who prefer care in Spanish and English<\/li><\/ul>\n<h2 class=\"wp-block-heading\">How the process works<\/h2>\n<ol class=\"wp-block-list\"><li><strong>Submit the referral.<\/strong> Fill out the form below or call us at (651) 403-6034. We only need basic contact information.<\/li><li><strong>We reach out.<\/strong> Our team contacts the person or family within 24 to 48 hours to explain services and answer questions.<\/li><li><strong>Assessment and plan.<\/strong> We coordinate the necessary assessment and verify eligibility (Medical Assistance or other programs).<\/li><li><strong>Care begins.<\/strong> We assign a caregiver who fits the person's needs, language, and schedule.<\/li><\/ol>\n<h2 class=\"wp-block-heading\">What do you need to refer someone?<\/h2>\n<p class=\"wp-block-paragraph\">No complicated paperwork is needed. A name, phone number, and a brief description of the need is enough to get started.
